Nature and scope
Provides personal care to residents as required by State Regulations, as outlined in the residentlan of care, educational and certification standards and as defined through Affinity Senior Living policy and training.
This job description is developed for the Supervisor In Charge. A supervisor is responsible for the direct supervision of personal care aides, including those who administer medications, to assure that care and services are provided to residents by personal care aides in a safe and secure manner and according to licensure rules. This involves observing personal care aides in the performance of their duties; instructing, correcting and consulting with aides as needed; and reviewing documentation by aides.
Minimum eligibility requirements
Successfully complete training as required or accepted by State regulations.
Be 21 years or older.
Be a high school graduate or certified under the G.E.D. program, or have passed an alternative examination established by the Department.
Have at least six months of experience in performing or supervising the performance of duties to be supervised during a period of three years prior to the effective date of this Rule or the date of hire, whichever is later, or be a licensed health professional or a licensed nursing home administrator.
Meet the same minimum training and competency requirements of the aides being supervised.
Earn at least 12 hours a year of continuing education credits related to the care of aged and disabled persons in accordance with procedures established by the Department of Health and Human Services.
Ability to read, write and speak English.
Ability to communicate effectively with residents, families, staff, vendors and the general public.
Be familiar with the Adult Care Home Residentsll of Rights and be willing to assist residents in exercising their rights.
Must have compassion for and desire to work with the elderly and understand that for each resident the facility is considered the Residentome.
Must demonstrate the ability to work responsibly as a team member as well as an individual.
Must be honest, ethical, fair, dependable, respect confidentiality and the rights and privacy of others.
Must practice and promote Affinity Guidelines, Policies and Procedures, Mission Statement, values and goals.
Maintain practice certification and required hours of Alzheimernd related dementia-specific training per year.
Exceptional organizational skills.
Must meet all health requirements.
Must pass criminal background check.
Must be able to perform duties and responsibilities (essential job functions), with or without reasonable accommodation.
Essential functions Ensure that staff can apply the homeccident, fire safety, disaster, elopement and emergency procedures for protection of the staff and residents.
Participate in resident and family care conferences as designed by the RCMr Executive Director.
Serve as liaison between resident, family, physician and other health care providers regarding health care issues, in RCMbsence.
Ensure that changes in resident conditions are evaluated, reported, monitored, and documented with follow up to assure interventions are implemented and documented.
Perform care procedures within the scope of practice
Ability to adhere to company policies and procedures and maintain confidentiality, professional appearance, and attitude; communicate effectively with other departments, residents, and visitors.
Responsible for supervising, assigning significant overall duties and tasks and responsibly directing the work of all care employees with full accountability for the performance of subordinates on designated shifts.
Responsible for the supervision of staff in the absence of Executive Director and RCM.
Effectively recommends hiring, performance evaluations, terminations, and discipline to include suspension pending investigations.
Working conditions (travel, hours, environment)
Scheduled hours; possible overtime. May work in other positions temporarily when necessary.
Is involved with resident, personnel, and visitors under all conditions and circumstances.
Is subject to callback during emergency conditions (e.g., severe weather, evacuation, post-disaster, etc.).
Is subject to injury from falls, burns from equipment, odors, etc., through the workday, as well as to reactions from dust, disinfectants, tobacco smoke, and other air contaminants.
Is subject to exposure to infectious waste, diseases, conditions, etc., including TB and the AIDS and Hepatitis B viruses.
May be subject to the handling of and exposure to hazardous chemicals.
Physical/sensory requirement
Medium work: ability to exert 20-50 pounds of force occasionally, and/or 10-25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to lift, carry, push, pull, or otherwise move objects.
Must function independently and have flexibility, personal integrity, and the ability to work effectively with residents and staff.
Must be able to relate to and work with the ill, disabled, elderly, emotionally upset and, at times, hostile people within the community.
