JOB SUMMARY

The Clinical Supervisor, under the direction of the Home Care Nurse Manager, is responsible for the day-to-day clinical activities of the Home Care multidisciplinary team. Ensures that staff provides high quality, accessible, cost-effective and patient-centered care according to acceptable clinical and agency standards of practice. Ensures compliance with local, state, and federal regulations and Joint Commission standards. Assists in the coordination of home health services provided to Home Care patients through documentation review and managing clinical workflow tasks in the Home Care Electronic Health Record (EHR). Collaborates with Home Care leadership in the development and implementation of departmental goals, financial performance, patient safety and performance improvement activities.

MINIMUM QUALIFICATIONS

Education: Graduate from an accredited school of nursing.

Experience: Minimum of three (3) years of recent relevant clinical experience; verifiable clinical competence.

Licensure/Certification/Registration: Current license in Minnesota to practice as a registered nurse; Wisconsin RN license within ninety (90) days of employment. Certification in associated specialty area of nursing or obtains within one (1) year of meeting eligibility requirements. BLS Certification with a designation of either BLS Provider or Healthcare Provider from either the American Heart Association or American Red Cross or Military Training Network. Wisconsin Background Study upon hire and renewed per Wisconsin state law.

PREFERRED QUALIFICATIONS

Education: Baccalaureate Degree in Nursing or its equivalent in a related field.

Experience: Minimum of three (3) years nursing experience to include skilled Home Care experience. Previous supervisory experience.

Licensure/Certification/Registration: Outcome and Assessment Information Set (OASIS) Certification.

Ability to compute rate, ratio, and percent and to draw or interpret bar graphs to apply concepts of basic algebra and geometry.

REASONING ABILITY - Intermediate Skills: Ability to apply common sense understanding to carry out instructions furnished in written, oral, or diagram form. Ability to deal with problems involving several concrete variables in standardized situations.
