1. Provides leadership, direction and supervision to the Social Work staff, ensuring continuity and consistency of participant care planning across all centers.
2. In partnership with the Social Work Lead Council and Program Manager, ensures that social work staff are effectively performing and participating on the Interdisciplinary Team for participant care and coordination, by overseeing competencies requirements, and providing ongoing coaching, supervision and training.
3. Advances the goals and objectives of the organization by developing, implementing, and supporting department specific performance goals, best practices and quality improvement programs.
4. Participates in establishing and monitoring utilization targets and financial targets, and working collaboratively with other managers on issues related to quality programming, efficiency and cost effectiveness.
5. Sets standard criteria and process, and performs utilization review of participant care plans.
6. Ensures delivery of quality services by overseeing the departmentompliance with regulatory requirements, adherence to policies and procedures, and leading continuous process improvement and risk management efforts.
7. In collaboration with Program Manager, provides input on Social Worker performance evaluations and setting of goals.
8. Provides supervision to direct reports (i.e. float staff).
9. Coordinates social work coverage at all centers in partnership with Social Work Leads and Program Managers.
10. Provides individual case management to participants as needed.
11. Provides after hours on-call coverage, as appropriate.
12. Organize and facilitate department meetings.
13. Supports, guides and participates in Social Work Lead Council.
14. Oversees social work related grants and the caregiver program and department specific projects.
15. Assists Program Managers with screening, hiring, and orienting of new Social Workers.
16. In collaboration with Program Manager and Learning and Development Department, coordinates and assigns training, supervision, and evaluation of student placements.
17. Establishes, develops, and maintains working relations with educational institutions and community agencies and organizations.
18. Provides oversight of contracted LCSW clinical supervision staff as applicable.
19. If LCSW, provide LCSW supervision hours to social workers as needed.
20. Serves as an effective link with On Lokdministration, OLSHSanagement team and department staff through effective communication, productive collaboration, demonstrated leadership and facilitation of change opportunities.
21. Other related duties as assigned.
QUALIFICATIONS (knowledge, skills, abilities)
1. Masteregree in Social Work or health related field. LCSW preferred.
2. Four years experience working as a clinical social worker, preferably in the field of geriatrics.
3. Two years experience in management/supervision/financial management.
4. Skilled facilitator and problem solver with excellent communication and interpersonal skills.
5. Proficient in Microsoft Word, Excel, Outlook, PowerPoint.
6. Experience with electronic health record system preferred.
