JOB SUMMARY: Supports clinical team in the High Fidelity Wraparound referral and enrollment process. Coordinates with families, stakeholders and HFWA providers for processing referrals and enrolling members into the program.
ESSENTIAL FUNCTIONS: Supports the referral process for enrollment of the High Fidelity Wraparound process. Supports the clinical team in the collection of assessments, data and enrollment of members.
Provides information to members and providers regarding HFWA services.
Assists in the adhoc process.
Participates in QI activities, including data collection, tracking, and analysis.
Assumes responsibility for self-development and career progression. Proactively seeks and participates in ongoing training (formal and informal) in all aspects of the Care Manager role.
Assists in the mentoring and training of new team members. Remains responsible for updating self on ever changing information to ensure accuracy when dealing with members and providers.
Contributes to process improvements to enhance achievement of personal, team and CME goals.
MINIMUM REQUIREMENTS: Education: Master's Degree
Experience: 1-3yrs

Licenses, Certifications, etc.: Meets Magellan credentialing requirements for independent professional practice IF performing TRF reviews (Masters level licensed behavioral health professional.)
Other: Understanding of plan benefit structures, psychiatric/medical terminology, and local communication resources; excellent verbal and written communication; able to multitask.

Responsibilities Licensure is required for this position, specifically a current license that meets State, Commonwealth or customer-specific requirements.

Minimum 2 years experience post degree in healthcare, behavioral health, psychiatric and/or substance abuse health care setting.
Strong organization, time management and communication skills.
Knowledge of utilization management procedures, mental health and substance abuse community resources and providers.
Knowledge and experience in inpatient and/or outpatient setting.
Knowledge of DSM V or most current diagnostic edition.
Ability to analyze specific utilization problems, plan and implement solutions that directly influence quality of care.
