General Summary: Under the general supervision of the Clinical Nursing Coordinator and/or the Practice Manager or Provider, provides safe, therapeutic and efficient care and services to patients. Assists in the care of patients, performs treatments, administers medications (if qualified and appropriately supervised), and instructs patients and their families in appropriate care, as directed by their provider.
Duties and Responsibilities: 1 Obtains and documents reason for visit, patient vital signs, including temperature, blood pressure, weight, respirations, pulse and BMI.
2 Assists providers with examinations, procedures and other processes related to direct patient care.
3 Collects specimens as directed by provider. Uses appropriate equipment and protocols for collecting, handling, testing, and preparing for transportation to lab including proper identification, storage procedures and techniques.
4 Ensures all patient care areas are adequately stocked with unexpired supplies and medications.
5 Prepares patients for scheduled procedures by assuring patient is appropriately gowned and instruments and supplies are set-up. Uses Universal Precautions. Sterilizes instruments, equipment and supplies as required. Handles and stores instruments, equipment and supplies appropriately.
6 Arranges for patients ordered diagnostic and therapeutic services by completing necessary paperwork and scheduling of appointments.
7 Maintains patient records to ensure an accurate record of the patients medical treatment.
8 Under the direct supervision of a licensed nurse or provider, prepares and administers vaccines, medications and controlled substances/narcotics as prescribed by the provider and as certified competencies permit. Asks prescribed screening questions, identifies risks and contraindications, questions and observes for appropriate responses.
9 Documents and processes prescription requests and refills per provider order.
10 Performs basic office procedures, examinations and/or tests ordered by provider and as certified competent (i.e. EKGs, ear lavage, vision tests etc.).
11 Provides health care information materials and reviews with patient and family members as directed by providers (i.e. VIS sheets etc.).
12 Under the direction of the provider, tracks, reviews and monitors follow-up of diagnostic test results. Relays information to patient from the provider regarding results and follow-up instructions.
13 Obtains comprehensive health information from patient/family members to review with providers or clinical resource at the time of the visit or over the phone. Uses this information to assist providers in determining the urgency for care or an alternative venue for care for patients with acute problems, should access not be readily available at the practice. This may include review of systems and history of present illness and/or past medical and social history. Documents in the patient record under the direction and supervision of the care provider.
14 Provides standard patient care instruction and education as provided and directed by the care team. Completes education on health literacy and foundations of patient education.
15 Completes pre-visit planning and visit prep according to established guidelines and providers standing orders for the chronic and preventative healthcare needs of the patient.
16 Efficiently directs and manages daily patient flow to manage fluctuating patient volumes and demands.
17 Effectively prioritizes daily patient care tasks and activities that are delegated by the care team.
18 Supports the care team by functioning as a health coach for disease prevention, early screening and testing and treatment and for helping patients manage chronic diseases.
19 Supports the provider during the visit and patient exam by collecting and entering information in the patient record, carrying out orders and completing post visit patient care education and instructions.
