PHI is seeking a full-time Program Manager II for The Health Trust. This position is located in San Jose, CA.
Under the direction of the Director of Strategy at The Health Trust, the Program Manager II helps provide strategic direction, as well as project management for the organization's food access strategies. This includes its Food For Everyone portfolio, as well as medically tailored meal and food pantry programs. These programs are all designed to build health equity by reducing food insecurity for Santa Clara County's most vulnerable residents, including low-income seniors, individuals who are homeless or recently housed, and individuals with complex chronic conditions. The Program Manager II supports the strategic leadership and management, partnership development, evaluation, communication, and fiscal management of these diverse food access strategies.

Duties & Responsibilities Leadership and Management:
- With support from the Director of Strategy, leads the strategic development, planning, evaluation, and implementation of food access projects for vulnerable populations. Ensures that ongoing programs are aligned with organization's overall strategic plan.
- Provides direction and oversight for budget development on project specific budgets related to both new and existing contracts.
- Provides proactive leadership in the development, implementation, and evaluation of sustainable programs. Analyzes programs and adjusts strategies and tactics to increase effectiveness.
- Manages program implementation to ensure goals and objectives are met.

- Convenes team members regularly to review work plans, share progress, identify barriers, and problem-solve issues.
- Provides supervision for subordinate staff, including but not limited evaluating performance, mentoring and coaching, making recommendations on employment actions and providing feedback.
Partnership Development:
- Develops and maintains partnerships with public and private organizations to support and advance the goals of The Health Trust. Organizations may include non-profit agencies, governmental agencies at the local, state, and national levels, food and retail industry members, advocacy organizations, and other entities with goals that align with The Health Trust.
Evaluation and Communications:
- In partnership with the Director of Strategy, develop and implement evaluation tools to track and report on unmet food access needs, and the impact of The Health Trust's food access strategies. Develop evaluation materials that demonstrate The Health Trust's impact, and distribute to local electeds, City/County government, and other stakeholders. Where appropriate, support publishing of key evaluation findings and sharing at national conferences (i.e. Grantmakers in Health, American Public Health Association annual conference, etc).
- Utilizing GIS analysis, identify gaps in food access and target sites for new programs. Create visually compelling maps to track impact of The Health Trust's work.
- In partnership with the Communications team, ensure the development of compelling communications materials (newsletter, fact sheet, policy updates) that are effective in conveying the impact of The Health Trust's work.
- Represent The Health Trust in meetings with partners and stakeholders, including public comment sessions held by local and state elected offices and government departments.
Development/Fundraising:
- In partnership with the Development Team, identify resource and funding opportunities at the local, state, and national level. Develop and submit funding proposals.
- In partnership with the Senior Vice President of Programs, identify and develop pitches to potential fee for service payers, including health plans.

Minimum Qualifications To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill, and/or ability required.
- Bachelor's degree from an accredited college or university in public health, nutrition, urban planning, public policy, or related field required. Master's degree preferred.
- Minimum of 7 years management and/or planning experience working in the non-profit sector. Experience working in the food access, healthcare, or housing sectors preferred.
- Strong ability to think strategically, particularly in relation to policy and programmatic tactics that result in desired changes.
- Strong leadership and organizational skills, and ability to effectively manage a small program staff.
- Experience with programs promoting health and nutrition in low resource communities that serve diverse audiences.
- Experience with program evaluation, grant-writing, and stewardship.
- Familiarity with state and local government programs and policies especially related to food security, affordable housing, and regional governance preferred.
- Policy analysis skills, including the ability to develop and evaluate policies and make policy recommendations.
- Experience with GIS strongly preferred.
- Qualitative research skills, including the ability to synthesize existing research, design data collection mechanisms, conduct literature reviews, and write reports.
- Ability to develop and manage work plans, and track progress.
- Strong writing and editing skills, including experience developing policy reports, communicating with external stakeholders, and incorporating feedback into work products.
- Strong oral communication skills.
- Excellent ability to plan and facilitate meetings, build consensus, and work collaboratively.
- Ability to arrange meeting logistics, take detailed meeting notes, and develop meeting summaries and next steps to share with participants.
- Proficient in computer basics, including experience with Microsoft Word, Excel and Power Point.
- Team-player and able to work cooperatively and effectively with a team to set goals, resolve problems and make decisions that promote program effectiveness.
- Ability to multitask and work effectively to meet tight deadlines.
- Possess excellent verbal and written communication skills. Must be able to prepare and deliver presentations and prepare written reports, correspondence, proposals, and other materials for a variety of audiences.
- Demonstrated ability to earn the respect of government and non-profit staff and decision-makers.
- Familiarity with Santa Clara County preferred.
