Reporting to the Supervisor of Complex Care Management, assesses, plans, implements, coordinates, monitors, evaluates options/interventions and develops a comprehensive plan of care for the patients. Promotes the achievement of optimal clinical and resource outcome. Performs Care Management Function in the practice, participates in a primary care physician led team that takes collective responsibility for the continuum of patient care for a medium- high and high risk population. Performs ongoing communication and works to promote quality of care through collaboration with PCP, specialist and all service team members, patients and families. Works with the Care Management Division leadership team to ensure alignment with the strategic goals and visions. Is responsible for identifying, assessing, planning, coordinating, implementing, monitoring and evaluating options and services across the continuum of care for individuals in assigned case load. Health care services are coordinated on behalf of the patient to promote the delivery of care at the appropriate level of care which will optimize clinical outcomes and satisfaction with services. The care manager assures the designation of primary responsibility among team members to each aspect of the plan, avoiding duplication and fragmentation. The care manager assesses and intervenes to address the complex psychosocial and behavioral health issues and needs associated with population management and transition plans. Demonstrates the knowledge and skills necessary to provide services to the assigned patient population. Works with the Care Management Division leadership team to ensure alignment with the strategic goals and visions.
Qualifications REQUIRED QUALIFICATIONS: Bacheloregree in Social Work from a school or program accredited by the Council on Social Work Education is required; Masteregree in Social Work preferred.
Three to five years of experience caring for patients with progressively increasing responsibility. Chronic, complex illnesses adult and/or pediatric care management experience preferred. Broad knowledge of chronic illness and medical and community resources required.
Positive, empathetic approach and ability to communicate effectively to patients/ families with a variety of health issues, age groups and socioeconomic backgrounds.
Flexible work schedule ekend/holiday/on call rotation.
Requires the ability to successfully use independent judgment and make decisions and requires little supervision.
Shall comply with the Illinois or Michigan state licensing and certification requirements or jurisdiction(s) in which she or he practices.
Social Worker with a BSW who has completed 3 years of supervised clinical hours will be required to pass the examination for the practice of social work and obtain applicable license as awarded by the Department of Professional Regulation within the employeeesignated state (Illinois/Michigan) of practice. Within 1 year of completing specified supervised clinical hours, the employee will be required to pass the examination for the practice of social work and obtain applicable license as awarded by the Department of Professional Regulation within the employeeesignated state (Illinois/Michigan) of practice.
Social Worker with a BSW who has not obtained 3 years of supervised clinical hours will be given the opportunity to complete this clinical practicum required to sit for the examination for practice of social work. Within 1 year of completing specified supervised clinical hours, the employee will be required to pass the examination for the practice of social work and obtain applicable license as awarded by the Department of Professional Regulation within the employeeesignated state (Illinois/Michigan) of practice.
Social Worker with an MSW who has not obtained their license will be required to pass the examination for the practice of social work and obtain applicable license as awarded by the Department of Professional Regulation within the employeeesignated state (Illinois/Michigan) of practice. Within 1 year of completing specified supervised clinical hours, the employee will be required to pass the examination for the practice of social work and obtain applicable license as awarded by the Department of Professional Regulation within the employeeesignated state (Illinois/Michigan) of practice.
Licensed Clinical Professional Counselor (LCPC) license also considered.
CPR Certification within 90 days of hire.
Care Management certification as appropriate for adult or pediatric within 3 years of hire.
Working knowledge of a clinical EMR system and Microsoft Office applications.
Able to work independently and efficiently with excellent time management skills.
Work requires substantial interpersonal skills to develop and maintain cooperative working relationships with personnel from other departments.
