This Full Time opportunity is for Nurse Practitioner or Physician Assistant within the Internal Medicine office in Zephyrhills. Typical schedule is Monday-Friday, day shift only. The role is responsible for working collaboratively with the physicians to manager groups of patients to improve access, patient outcomes, and promote evidence-based medicine. Previous geriatric nursing experience preferred.
Responsibilities - Conduct the Welcome to Medicare visit and Annual Wellness visit
- Provide services to enhance care transitions from in-patient and/or skilled nursing facilities under the direction of the PCP
- Manage populations of patients by promoting health initiatives
- Perform incidental duties as needed
Requirements - ARNP or PA with valid Florida license and national certification
- Excellent communication and problem solving skills
- Strong customer service orientation
- Geriatric nursing experience highly preferred
