Job description: JOB DESCRIPTION Reporting to the Nurse Manager / Nurse Clinician, you will be responsible for delivering patient-centric and compassionate care according to patients' needs and condition.
Job responsibilities: » Monitor, reassess and evaluate patient's condition implementing a care plan for the patient and support family members to cope with the patient's advanced illness.
» Communicate and build rapport with patients and their families.
» Maintain proper documentation of patient's condition and accurate collation of data.
» Perform nursing care, such as medication administration and wound dressing as required.
» Follow up to ensure continuity of patient care and treatment plans.
» Maintain a high degree of confidentiality and discretion in matters involving patient care.
Requirements: » Minimum a Diploma in Nursing and registered with Singapore Nursing Board
» Preferably with a minimum of 2 years of post-registration practice in healthcare or hospice setting
» Able to execute 3 rotating shift tasks
» Good interpersonal and communication expertise
» Meticulous and accurate
» Willing to contribute as a capable team player
» Able to execute under pressure
