Job description: JOB DESCRIPTION You will be part of the Population Health and Community Transformation team providing psychosocial assessment and intervention for identified residents with/without medical condition(s) in the northern community.
Your function will specifically support the Assertive Intervention Pilot Project for top 60 frequent A&E patients through proactive interventions to address their psychosocial issues in the community. You will be expected to collaborate with other care providers and community resources in the broader ecosystem such as primary care and social partners in addition to our medical and nursing team to reduce episodic, unconnected care for residents and optimise resources for the healthcare system. The aim is to proactively engage these residents, as well as build alternatives to enable them to self-manage in the community.
» Degree or Postgraduate Diploma in Social Work required
» Preferably 4 years of social work practice
» Must be a team player who enjoys working in community setting
» Able to work with minimum supervision
» Being a registered member with the Singapore Association of Social Workers
» Open for renewable contract employment
