Job description: JOB DESCRIPTION Broad Function
supervise the development of sector (centre-located), so that service providers are able to provide quality client care. This is done in collaboration with the Ministry of Health, industry partners and stakeholders and in response to the growing needs of an ageing population.
Key Responsibilities and Desired results
» Sector Development for centre-located care:
» To explore, develop and enhance new models of community-located eldercare and to build key enablers that integrate care and facilitate client transitions.
» Raise consciousness of financial schemes available to support community in using LTC services
» Deployment of Live-in caregiver by LTC service providers under FDW scheme
» Identify current gaps along the care continuum and review international practices of community-located eldercare
» Review middle earnings group's expectations on LTC services
Understand expected expenditure on LTC services
» Analyse data from ILTC & IRMS portal, SGO, and other sources on ILTC service utilization and take up
» Manage/review programmes and services related to centre-located care
» Accreditation of centre-located services
» Collaborate with potential service providers to actualise new models of care, with focus on operational needs
» Track progress and ensure compliance to timelines in project development and implementation
» Facilitate project evaluation and efforts in sustaining and spreading prosperous models of care
» Account Management
» Account manager for private and a few VWO operators (supported by executive)
» Service provider engagement on performance management, service gaps, engagement on new pilots/initiatives/projects
» Management of feedback from MOH/AC hotline/advisors/public on service provider
Job Requirements: » Diploma/Degree in Nursing, Social Work or Allied Health disciplines or Bachelor of Science with healthcare-related module and practice
» significant practice working with service providers in the acute care or long term care setting in Singapore
» Computer literacy with proficiency in MS Office applications.
» Proficiency in basic statistics
» capability to use local languages and dialects is an advantage
» expertise and interest in geriatrics and community services
Critical thinker and self-directed
Competency in interpersonal and communication expertise
