Job description: JOB DESCRIPTION » Provide end to end Casework/ Psychosocial Care support and appropriate level of care through assessment, care planning, interventions and evaluation of complex/specialized cases.
» Conduct financial assessment, field visit, build good rapport with community agencies or individuals to mobilize social resources to meet resident’s need, to ensure continuity of care.
» Coordinate with Rehab team for home modification work for resident as part of the discharge care plan.
» Facilitate residents and caregivers support group networking sessions.
» Maintain the right documentation for case management and audit purposes. Requirements
» Degree or Master in social work is required
» Being a registered member with the Singapore Association of Social Workers
» At least 3 years of social work practice preferably in clinical setting will be an advantage.
» Strong advocate for holistic and person-centred approach to care for residents
» Individuals with good clinical practice will have added advantage
» practice in case management, patient education, data management and utilization review will be an advantage
» Matured and Confident in dealing with residents, family members and the public
» Possess strong interpersonal and good communication expertise

History
Founded in 1910 by a group of Cantonese merchants who wanted to provide Cantonese immigrants with free medical services, Kwong Wai Shiu is one of Singapore’s oldest charitable healthcare institutions. Through the Kwong Wai Shiu Free Hospital Ordinance, a 6-acre piece of land with 3 colonial buildings was parceled off from what was then Tan Tock Seng Hospital, and given to us to run for 102 years.
Initially, Kwong Wai Shiu Hospital’s in-patient services were only available to the Cantonese, although out-patient services were accessible to all, but in 1974, the Constitution of Kwong Wai Shiu Hospital was expanded to include the admission of people of all races into our in-patient facilities. While holding strongly on to our traditional values of compassion and care, we at Kwong Wai Shiu Hospital also endeavor to keep up with the ever-changing healthcare needs of Singaporeans.
